Change of Major, Dual
Degree, Minor Request form

Students must submit a Change of Major, Dual Degree, Minor Request form to initiate an audit to determine if they have met the requirements to declare or change their major, second degree, concentration, or a minor. This request may be submitted in advance of the end of the semester in which students are completing their final declaration of major courses and/or earn the grade point average required to declare. These requests will be processed once grades for the current semester have posted. See your catalog of graduation for specific declaration of major requirements.

All students must apply for approval to register for capstone course(s): MGT 4893, MKT 4893 and ACC 4163. Students should read the application instructions, print and sign the form, and submit it to the Advising Center front counter.  
Early submission is highly recommended to assure the pre-audit and capstone course approval can be completed before registration begins.  
Submit form the February before graduation for Summer or Fall graduation, and the September before graduation for Spring graduation.    
Students will be notified by e-mail regarding the disposition of their application.
